Our model of care is designed to complement the care and support provided by a health care team, hospital or GP. MPAs model of care and telehealth nursing navigation delivers flexible, nurse‑led support where needed, with a tiered approach ensuring the right care at the right time—strengthening the nurse–patient relationship and valuing the role of peer support and lived experience.

Patients can be referred at any time, from diagnosis onwards, particularly at key transition points or when additional supportive, emotional, or practical care needs arise.
